The skin is home to billions of microorganisms, from bacteria, viruses and fungi, their gene and metabolites. Why is acne taking over my skin?
Until recently, acne was believed to be caused by the colonisation of bacteria, most conventionally Cutibacterium acnes (Propionibacterium acnes), of the sebaceous follicle, creating a non-inflammatory comedo to an inflammatory nodule. New evidences has emerged that the signs of acne already started even before comedo is formed. The causes of acne is mostly multifactorial and very complex.
New studies have shown that C. acnes is a significant skin commensal in both acne and acne-free individuals. Researches also found that amongst this, C. Acne is much more complicated than what we thought previously. They have found different sub-strains. They suspect that the acne-causing culprits are the ones with virulence factors such as lipase, protease, hyaluronate lyase, endoglycoceramidase, neuraminidase, and Christie–Atkins–Munch-Petersen (CAMP). Lipase attracts neutrophils and hydrolyzes sebum triglycerides to free fatty acids, inducing inflammation and hyperkeratosis.
Protease, hyaluronate lyase, endoglycoceramidase, and neuraminidase break down the extracellular matrix, which aids the attack of C. Acne. As the extracellular matrix dissolves, inflammatory cells (i.e., neutrophils, monocytes, dendritic cells,) penetrate the hair follicle, bringing about the movement of bacteria, keratin, and sebum to the dermis, which evokes foreign body granuloma and scarring.
